Ελληνικά - Ειδική Δήλωση Ιδιωτικότητας – Πανελλήνιος Σύλλογος Γυναικών με Καρκίνο Μαστού «Άλμα Ζωής»

1. Παρέχω την πλήρη, ενημερωμένη, ελεύθερη, συγκεκριμένη και ρητή συγκατάθεσή μου στον Πανελλήνιο Σύλλογο Γυναικών με Καρκίνο Μαστού «Άλμα Ζωής», στο Think Pink Europe και στα υπό την άμεση εποπτεία τους πρόσωπα, προκειμένου άπαντα τα προλεχθέντα πρόσωπα να κάνουν χρήση οποιασδήποτε φωτογραφίας, βιντεοκασέτας, ηχητικής κασέτας ή άλλης εγγραφής της εικόνας ή και της φωνής μου κατά τη διάρκεια του Αγώνα, με σκοπό τη δημιουργία αρχειακού βίντεο/υλικού για τον οικείο Αγώνα, καθώς και τη δημιουργία προωθητικής καμπάνιας για τους επόμενους Αγώνες. Τα ως άνω αναφερόμενα προσωπικά δεδομένα μου θα διατηρούνται για διάστημα 5 ετών μετά την πραγματοποίηση του οικείου Αγώνα, σύμφωνα και με τον ανωτέρω περιγραφόμενο σκοπό χρήσης τους, κατόπιν δε τούτου θα καταστρέφονται κατά τρόπο που θα προσιδιάζει στο υλικό ενσωμάτωσής τους.
2. Συγκατατίθεμαι πλήρως, ελευθέρως, συγκεκριμένως και ρητώς στη χρήση και επεξεργασία των στοιχείων ταυτότητας που δηλώνω στη φόρμα εγγραφής από τον Πανελλήνιο Σύλλογο Γυναικών με Καρκίνο Μαστού «Άλμα Ζωής» και τα υπό την άμεση εποπτεία του Συλλόγου αυτού πρόσωπα, με σκοπό την ενημέρωσή μου για τα νέα και τις δράσεις του Συλλόγου, καθώς και την υπ’ αυτού πρόσκλησή μου σε δραστηριότητες που θα διοργανώνει, δια της αποστολής εις εμέ σχετικών μηνυμάτων ηλεκτρονικού ταχυδρομείου ή/και της πραγματοποίησης τηλεφωνικών κλήσεων προς τον αριθμό τηλεφώνου μου. Το προσωπικό δεδομένο υγείας μου που αφορά σε εμπειρία καρκίνου μαστού που έχω τυχόν βιώσει, το οποίο δηλώνεται στην παρούσα φόρμα, δηλώνεται προς εξυπηρέτηση του σκοπού συμμετοχής με ροζ μπλουζάκι, που σκοπεί, περαιτέρω, στην αποτελεσματικότερη ευαισθητοποίηση του κοινού αναφορικά με το ζήτημα του καρκίνου του μαστού, καθώς και στην πιο εκτεταμένη και αποτελεσματικότερη πρόληψη από πλευράς των γυναικών. Το προσωπικό αυτό δεδομένο που αφορά στην υγεία μου, όπως και την τυχόν συμμετοχή μου σε ομάδα που εγγράφεται ομαδικώς στο Greece Race for the Cure® στον Aγώνα των 5 χλμ ή στον περίπατο 2 χλμ στον οποίο θα συμμετάσχω θα διατηρούνται για διάστημα 5 ετών καθώς επίσης τα στοιχεία που αφορούν στην ηλικία μου, το μέγεθος της μπλούζας με την οποία θα τρέξω θα διατηρούνται μόνο για το διάστημα διεξαγωγής του αγώνα, κατόπιν τούτου, θα καταστρέφονται.
3. Έχω το δικαίωμα να γνωρίζω ποια δεδομένα προσωπικού χαρακτήρα που με αφορούν αποτελούν ή αποτέλεσαν αντικείμενο επεξεργασίας και χρήσης, να προβάλλω οποτεδήποτε αντιρρήσεις ως προς την λόγω επεξεργασία και χρήση, να αιτηθώ τη διαγραφή των προσωπικών δεδομένων μου και να ανακαλέσω ανά πάσα στιγμή τη συγκατάθεσή μου ως προς την περιγραφόμενη στις προηγηθείσες υπ’ αριθ. 1 και 2 παραγράφους του παρόντος εγγράφου επεξεργασία και χρήση προσωπικών δεδομένων μου. Τυχόν τοιαύτη ανάκληση από μέρους μου δεν θα έχει αναδρομικό αποτέλεσμα, αλλά θα ισχύει μόνο για το μέλλον, ώστε κατ’ αποτέλεσμα δεν θα καθίσταται εκ των υστέρων παράνομη η ήδη συντελεσθείσα έως την ανάκληση επεξεργασία και χρήση των προσωπικών δεδομένων μου.
4. Για την άσκηση των ανωτέρω δικαιωμάτων μου, γνωρίζω ότι μπορώ να απευθύνομαι στον Υπεύθυνο Προστασίας Δεδομένων, κύριο Γιάννη Καλαντζάκη που έχει οριστεί ως DPO από τον Πανελλήνιο Σύλλογο Γυναικών με Καρκίνο Μαστού «Άλμα Ζωής». Περαιτέρω, ενημερώθηκα ότι έχω το δικαίωμα υποβολής καταγγελίας στην Αρχή Προστασίας Δεδομένων Προσωπικού Χαρακτήρα, αν θεωρώ ότι προσεβλήθη κάποιο από τα δικαιώματά μου ή δεν ικανοποιήθηκε προσκηκόντως.

The hosting provider’s servers

The hosting provider has the ISO 27001: 2013 standard, which originated from the "Code of Practice for Information Security Management" (BS7799). This refers to a management system specifically for information security (Information Security Management System ISMS) and specifies how you can demonstrably manage security risks. As an ISMS interpretation, the ISO 27001: 2013 standard specifies that you define a scope and policy, perform a risk analysis, select measures for the identified risks, implement and manage (allocate resources) and finally (cyclically) monitor and review the above.

The data centres are physically housed in Ostend. Our internal systems are located at our head office (Ter Zwaanhoek 34, 8400 Oostende).

For what purposes are these data used?

The use of the various personal data depends on application to application.For the Race for the Cure data, the personal data will be used for contacting participants and informing them about the activities of Think Pink Europe and the national breast cancer charity they have supported.

Are cookies stored as well?

Identification cookies are used to identify visitors to use the website management application. Functional cookies ensure that the website works properly. For example, they help you remember your login information or language preference. Only the administrator has access to these data in order to be able to identify potential problems.
